Coronary Heart Disease (CHD)

The Coronary Heart Disease Risk Assessment uses simple questions about your health, your family history, and your lifestyle to determine the chance or probability of you developing the disease. It doesn't mean you will. What it does mean is that current scientific research indicates that the health characteristics you exhibit today when answering these questions are shared by persons who develop coronary heart disease.

What is your age?
Family History
Do you have a close relative who has had a heart attack?
If yes, did your close relative have a heart attack before age 65?
Do any of your close relatives have high blood pressure?
Do you or any of your close relatives have diabetes?
Do you or any of your close relatives have high cholesterol?
Diet & Lifestyle
Do you eat 5 or more servings of fruits and vegetables every day?
Do you eat whole grains, nuts and fiber every day?
Do you smoke?
Are you overweight?
Is your waist circumference greater than 40" (male) or 35" (female)?
Do you have high blood pressure?
Do you exercise 2-3 times per week for at least 1 hour (i.e. vigorous walking, biking, hiking, aerobics)?
